Willamette Valley Behavioral Health ("WVBH," "we," "our," or "us") is committed to protecting your privacy. This Privacy Policy explains how we collect, use, and safeguard information when you visit willamettevalley-bh.com (the "Site").

  • Coordinate care and verify insurance benefits

  • Comply with legal and regulatory obligations

We will not use your information for marketing purposes without your explicit consent.

Please note: our contact and intake forms are processed through a third-party service (Web3Forms). While we take reasonable steps to protect your data, Web3Forms is not a HIPAA Business Associate. We recommend you do not include sensitive protected health information in the initial contact form. Detailed clinical intake information will be collected through HIPAA-compliant channels at or before your first appointment.

Your Rights

Depending on your location and applicable law, you may have the right to:

  • Access the personal information we hold about you
  • Request correction of inaccurate information
  • Request deletion of your information, subject to legal retention requirements
  • Withdraw consent for non-essential communications

HIPAA Notice of Privacy Practices

This Notice describes how medical and mental health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our Legal Duty

We are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of our legal duties and privacy practices. We are required to abide by the terms of this Notice currently in effect.

Payment: We may use and disclose your PHI to obtain reimbursement for services, including submitting claims to your insurance carrier and verifying your coverage.

  • Right to an Accounting: You may request a list of certain disclosures we have made.
  • Right to Restrict: You may ask us to limit how we use or share your PHI.
  • Right to Confidential Communications: You may ask us to contact you in a specific way or at a specific location.
  • Right to a Paper Copy: You may request a paper copy of this Notice at any time.

How to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized for filing a complaint.
